    PRIMARY DIAGNOSIS: AUTISM, INTELLECTUAL DISABILITIES
    SECONDARY DIAGNOSIS: SEIZURE DISORDER
    current age: 32 years

    ALLERGIES
    BEE VENOM – LIFE THREATENING. Administer Epinephrine 0.3mg/0.3ml injection syringe as needed for anaphylaxis.
    Dairy products (except eggs) – non life-threatening: causes gastrointestinal pain.
    Red dye – non life-threatening: causes hyperactivity and dysregulation.

    MEDICATIONS
    divalproex ER 500 mg tablet; 2x/day
    Commonly known as: DEPAKOTE ER

    risperiDONE 0.5 mg tablet; 2x/day
    Commonly known as: RisperDAL

    LORazepam 1 mg tablet; Take 1 tablet (1 mg total) by mouth as needed for severe agitation, and/or intense aggression or self injury that cannot be safely managed for longer than 2 minutes. Take 1 tablet by mouth 1 hour prior to medical/dental appointment. Do not exceed 2 doses in 24 hours.

    clonazePAM 0.5 mg tablet; 2x/day

    loratadine 10 mg tablet; 1 tablet once a day
    Commonly known as: Allergy Relief (loratadine)

    EQUIPMENT
    Wears a hockey helmet for protection from self-injurious behavior. Should be kept on at all times in an emergency, unless there is a head injury.

    HOW TO SUPPORT ME

    EXTREME BEHAVIOR CHALLENGES YOU MAY ENCOUNTER:
    * head banging on walls or other hard surfaces is an anxiety/fear response
    * biting himself or others is an anxiety/fear response
    * pinching or punching himself or others is an anxiety/fear response
    * self-injury can also include punching himself
    * screaming, pointing and saying “all done” or “goodbye”

    HOW TO RESPOND TO MY BEHAVIOR CHALLENGES:
    *Always use a calm, soft voice; supplement what you’re saying with pictures from your phone or sketches
    *tell me what’s happening using “first/then” like “first you lay down, then we check your heart (show a pic of a stethoscope).”
    *Nicky is familiar with medical tools and devices; show it to him first and ask “ok?” before using it. He should answer “ok” or “all done.” If he says “all done”, use the first/then strategy above: “first I take your blood pressure, then I’m done” and if available, follow with a food reward (cookie, cracker, etc: see more about food below).
    *please try to avoid physical restraint, as injury to others is increased. If he is self-injurious (only to himself), provide him with a blanket. He may destroy (rip) the blanket but it’s better than attacking someone.
    *do not shout or move quickly around him. He will not respond verbally with “yes” or “no” – don’t expect any verbal response (except referring to the bathroom per below).
    *tell him “it’s okay, you’re okay” frequently.
    *start a movie on a device of any kind. YouTube has all the movies he enjoys and can be quickly searched. Search on YouTube for Disney classic movies like: Aladdin, Little Mermaid, Lion King, Fern Gully, Rescuers Down Under.
    *or, play Disney music from those movies above (like from a music streaming service)
    *provide Nicky with food – he’s not picky, just no dairy. Granola bars, gummy candy, anything that requires a lot of chewing will slow him down.
    *he is fully continent but should be asked if he needs the bathroom. He will repeat “bathroom” if he needs it, or “all done” if he does NOT need it.
